摘要

摘 要心理韧性作为积极心理学领域的关键研究主题之一,与医务人员的心理健康密切相关,其作为一种内在保护性的心理资源,可通过外界干预来提升进而发挥保护性作用。目前我国在医务人员职业心理健康领域,缺乏相对易用、高效的医务人员心理韧性培育方案。本研究旨在了解医务人员心理健康现状,探究心理韧性对心理健康的保护作用及机制,结合心理韧性框架理论构建了一套针对医务人员的韧性培育方案;利用随机对照试验验证其有效性,为医务人员心理健康干预措施制定提供证据,从而提升医务人员心理健康水平。本研究包含三个子研究:研究一通过横断面问卷调查医务人员心理韧性、职业倦怠和抑郁现况,并探究三者之间关系;研究二结合文献回顾、实证证据与专家咨询制定了一套韧性培育方案;研究三通过随机对照试验探究方案有效性。研究使用了G-Power3.1软件计算样本量,以及SPSS23.0和AMOS24.0等软件进行数据分析。研究结果发现:(1)医务人员抑郁(29.80±11.01分)、心理韧性(3.25±0.79分)以及情感耗竭(18.42±12.92分)和个人成就感(15.10±11.56分)均处于中等水平,而去人性化程度较轻(5.54±6.26分);(2)不同性别医务人员在去人性化维度上差异具有显著性(t=2.241,p=0.027<0.05),且每月值夜班情况对三个变量均有显著影响(p<0.05);(3)心理韧性、抑郁和职业倦怠两两之间有显著相关性:抑郁与职业倦怠之间呈显著正相关关系(p<0.01),心理韧性与抑郁、职业倦怠之间呈显著负相关关系(p<0.01),且心理韧性在医务人员职业倦怠和抑郁之间发挥了部分中介作用(效应占比为40%);(4)随机对照试验中,试验组在干预前后各个变量上存在显著差异(p<0.05),心理韧性在干预后优于干预前,而对照组没有显著变化。医务人员可以通过心理韧性的保护作用来改善其职业倦怠和抑郁等心理健康问题。本研究初步证实了心理韧性培育方案有效提升了医务人员心理韧性水平、降低职业倦怠和抑郁,有进一步推广和应用的价值。关键词:医务人员;心理健康;心理韧性;职业倦怠;抑郁

AbstractAs one of the key research topics in the field of positive psychology, psychological resilience is closely related to the mental health of medical personnel. As an internal protective psychological resource, it can be improved by external intervention to play a protective role. At present, in the field of occupational mental health of medical personnel in China, there is a lack of relatively easy to use and efficient psychological resilience cultivation programs for medical personnel.The purpose of this study is to understand the current situation of mental health of medical personnel, explore the protective effect and mechanism of psychological resilience on mental health, and construct a set of psychological resilience cultivation program for medical personnel based on the psychological resilience framework theory. Randomized controlled trials were used to verify its effectiveness and provide evidence for the formulation of mental health intervention measures for medical personnel, so as to improve the mental health level of medical personnel.This study consists of three sub-studies: Study 1 investigates the current situation of psychological resilience, job burnout and depression of medical personnel through cross-sectional questionnaire, and explores the relationship among the three; The second study combined literature review, empirical evidence and expert consultation to develop a set of psychological resilience cultivation program; Study 3 explored the effectiveness of the program through randomized controlled trials. The G-Power3.1 software was used to calculate the sample size, and SPSS23.0 and AMOS24.0 software were used for data analysis.The results showed that: (1) the depression (29.80±11.01 points), psychological resilience (3.25±0.79 points),emotional exhaustion (18.42±12.92 points) and personal achievement (15.10±11.56 points) of medical personnel were at the medium level, while the degree of dehumanization was light (5.54±6.26 points); (2) There were significant differences in the dehumanization dimension among medical personnel of different genders (t=2.241, p=0.027 <0.05), and the monthly night shift had a significant impact on the three variables (p<0.05); (3) There was a significant correlation between psychological resilience, depression and job burnout: There was a significant positive correlation between depression and job burnout (p<0.01), and there was a significant negative correlation between psychological resilience and depression and job burnout (p<0.01), and psychological resilience played a partial mediating role between job burnout and depression of medical personnel (effect ratio was 40%); (4) In the randomized controlled trial, the experimental group showed significant differences in various variables before and after intervention (p<0.05), and psychological resilience was better after intervention than before intervention, while there was no significant change in the control group.Medical personnel can improve their mental health problems such as job burnout and depression through the protective effect of psychological resilience. This study preliminarily confirmed that the psychological resilience cultivation program can effectively improve the psychological resilience level of medical personnel and reduce job burnout and depression, and has the value of further promotion and application.Key words: Medical personnel; Mental health; Psychological resilience; Job Burnout; Depression
