我国人口老龄化不断加深，出于对老年人全周期健康情况的关注，我国医养结合政策、服务内容不断深化，目前国内对老年人医养需求及养老模式选择主要集中于机构养老、健康能力不足的老年人，较少对城市老年人及老年人养老模式选择进行研究。本文立足于老年人医养需求，为优化老年人整合照护服务体系提供支持。本文使用数据来源为清华大学银色经济与健康财富发展指数课题组全国老年人调研数据，使用单因素分析法、多因素分析法，探讨老年人医养需求及养老模式选择及其影响因素。研究结论：老年人医养需求中，倾向性因素中性别；使能因素中健康管理、锻炼情况、作息、养老意识及规划、城市级别、月均可支配收入、固定资产等；需求因素中慢病因素有意义；养老模式选择方面，倾向性因素中学历，使能因素中健康管理方式、饮食、体检习惯、预估养老花费、金融资产、保险有无；慢病因素有意义。根据研究结果提出建议：一、应当重视预防保健与体育锻炼的健康老龄化思想宣传；二、建立老年人健康能力评估体系；三、提升社区基本医疗服务可及性，加强地区医联体建设，由基层社区日常管理、风险预警，形成社区与医院的转运通路；四、发挥政府规范监督作用规范产业发展，引入市场主体；五、从实用角度出发，设计有层次的老年人整合照护服务体系，为不同能力阶段老年人提供适当、切实的服务内容

On November 18, 2021, the Central Committee of the Communist Party of China and the State Council "Opinions on Strengthening Aging Work in the New Era" (hereinafter referred to as "Opinions") put forward: "Integrate the concept of active and enterprising aging and the concept of healthy aging into the entire process of economic and social development". Since 2013, the policy and service content of the combination of medical care and elderly care in my country have been continuously deepened. Domestic pension medical needs and pension model choices are mainly concentrated in construction institutions. There is insufficient research on the correlation with pension model choice. The research object of this paper is the influencing factors of medical care needs and pension patterns of the elderly.Research methods: Based on Anderson's behavioral model and bibliometric analysis of the medical care needs of the elderly in my country, this paper uses the national medical care service demand survey data of the Silver Economic and Health Wealth Development Index Research Group of Tsinghua University. , and using univariate and multivariate analysis methods. , to construct a research framework from three dimensions of demand, inclination and enabling, to explore the influencing factors of the medical needs of the elderly and the choice of pension models.Research results: (1) Among the propensity factors such as medical care demand, gender, health management, pension awareness and planning; among the contributing factors such as monthly disposable income of the elderly, city level, fixed assets, and pension expenditure sources; among the demand factors , chronic diseases of the elderly and other factors are meaningful; (2) In the selection of pension models, education background, health management methods, diet, physical examination habits, expected pension costs, financial and contributing factors among the susceptibility factors, assets and insurance is meaningful.Conclusions and suggestions: 1. The comprehensive nursing foundation in my country is weak; 2. The elderly with good economic status and poor physical health have a large demand for medical treatment; 3. The elderly with high health attention, regular work and rest, and regular exercise, The demand for emergency medical care is high; 4. Educational background, economic status and health awareness are the main influencing factors for the choice of pension model. According to the research results, the following suggestions are put forward: 1. Pay attention to the propaganda of the concept of healthy aging; 2. Establish an evaluation system for the health capacity of the elderly; 3. Improve the accessibility of basic medical services in the community, strengthen the construction of regional medical alliances; supervise the introduction of market services The role of the main body; 5. Design a graded and practical comprehensive care service system for the elderly to provide appropriate and practical services for the elderly with different health abilities.
