摘要

随着经济社会的持续发展,一方面,建立基于各方利益的协同改革是“新医改”取得成功的基础,另一方面,当前在医疗卫生服务系统与其他系统之间,医疗卫生服务系统内部不同类别、层级医疗卫生机构之间都存在协同成本较高的问题。2020年新冠肺炎疫情爆发进一步凸显了我国医疗卫生服务领域的“两个依附”现象与“两个分离”问题。在供给侧改革的背景下本研究通过实证调研,运用质性和定量的混合研究方法,研究协同治理能力对公共组织绩效的主要影响因素和影响机制。本研究首先通过扎根理论的方式,分析了协同治理能力对公共组织绩效的主要影响因素。随后采用案例研究方法和过程追踪法对案例进行纵向剖析和横向比较,结合扎根理论研究和案例研究形成理论模型和研究假设。进一步,对理论框架进行实证检验,一方面采用Amos结构方程模型基于22家公共部门的2640名员工进行计量实证研究,分析协同治理能力对于公共组织绩效影响的主要因素,揭示我国医疗组织融合协同的基础;另一方面运用定性比较分析(QCA)方法分析协同治理能力对公共组织绩效的影响机制,研究设计推动协同治理能力提升的政策工具。通过以上实证研究,本研究得到以下研究结论:第一,协同目标战略能力、协同技术资源能力对公共组织绩效具有显著的正向作用。第二,协同制度建设能力、协同文化建设能力在协同目标战略能力、协同技术资源能力对公共组织绩效中发挥显著的中介作用。第三,协同目标战略能力{1} *协同制度建设能力{1} ,协同目标战略能力{1} *协同文化建设能力{1} ,协同技术资源能力{1}*协同文化建设能力{1} ,协同技术资源能力{1}*协同制度建设能力{1}是促使组织公共组织绩效提升的有效条件。第四,对于公共组织绩效的提升,需要根据组织资源禀赋来创造条件,如加强公立医院协同治理能力中巧实力的建设。本文的贡献主要有两个方面。第一,首次探究了中国情境下协同治理能力对公共组织绩效的影响因素和影响机制。一方面验证了协同目标战略能力、协同技术资源能力对公共组织绩效的直接影响,另一方面进一步深入探究了协同治理能力与公共组织绩效之间复杂的作用机制,即协同制度建设能力、协同文化建设能力发挥显著的中介作用,对于丰富协同治理研究、提高动态竞争环境中公共组织绩效具有理论意义和现实意义。第二,拓展了协同治理能力在医疗服务这个公共管理领域的研究场域,为探索中国特色的医疗卫生协同治理理论提供了有意义的参考。

With the sustainable development of the economy and society, on the one hand, the establishment of coordinated reforms is the foundation of the success of the "new medical reform"; on the other hand, between the medical and health service system and other systems, different types and levels of internal medical and health institutions all have high synergy costs. The COVID-19 outbreak in 2020 has further exposed the "two attachments" and "two separations" in China's medical and health services. In the context of supply-side reform, this research uses a mixture of qualitative and quantitative research methods through empirical research to explore the main factors and causal mechanisms of the impact of the organization’s collaborative governance capacities on the performance of public organizations.Based on the grounded theory, this research analyzes the main factors of collaborative governance ability on the performance of public organizations. Then, the case study method and process tracking method were used to analyze and compare the cases vertically and horizontally. Combined with further theoretical review of qualitative research, the preliminary framework of case study is theorized to form a complete research framework and research hypothesis. In the chapter of empirical analysis, Amos structural equation model is adopted to conduct quantitative empirical research based on 2,640 employees of 22 public sectors to analyze the influencing factors of collaborative governance capacities on the performance of public organizations to reveal the foundation of the integration and collaboration of medical organizations in China, and qualitative comparative analysis (QCA) is used to analyze the influencing mechanism of collaborative governance capacities on the performance of public organizations to design policy tools to promote the improvement of collaborative governance capacities.Based on the above empirical studies, the following conclusions can be drawn from this study: First, capacity to coordinate goals and strategies, capacity to coordinate technologies and resources have a significant effect on the performance of public organizations. Second, capacity to collaborative institutional-building and capacity to collaborative cultural-building play a significant mediating role in coordinate goals and strategies, capacity to coordinate technology and resource on the performance of public organizations. Thirdly, Capacity to coordinate goals and strategies {1}* Capacity to collaborative institutional-building {1}, Capacity to coordinate goals and strategies {1}* Capacity to collaborative cultural-building {1}, Capacity to coordinate technologies and resources {1}* Capacity to collaborative cultural-building {1}, Capacity to coordinate technologies and resources {1}* Capacity to collaborative institutional-building {1} are effective conditions to promote the performance improvement of public organizations. Fourthly, for the improvement of the performance of public organizations, it is necessary to create certain conditions in combination with their own organizational resource endowments, such as strengthening the construction of smart strength in the collaborative governance capacities of public hospitals.The contribution of this paper is mainly in two aspects. Firstly, it explores the impact and internal mechanism of collaborative governance capacities on public organization performance in the Chinese context. For one thing, it verifies the direct impact of capacity to coordinate goals and strategies, capacity to coordinate technologies and resources on public organization performance. For another, it further explores the complex mechanism between collaborative governance capacities and public organization performance, that is, collaborative institutional-building capacities and collaborative cultural-building capacities play a significant mediating role, which is to enrich collaborative governance research and improving public organization performance in a dynamic competitive environment. Secondly,it expands the research field of collaborative governance capacities in the field of public management of medical services, which contributes empirical evidence from Chinese medical organizations to the development of the collaborative governance theory of public management, and provides a meaningful reference for the exploration of the collaborative governance theory of medical and health with Chinese characteristics.
